For me and b/f it's really a decision of the moment. As for preferrence, we are truly 50/50, satisfied as long as we're doing "something" together but it seems we go in cycles. Not literally on top or bottom. It more generally is thought to mean who is "giving" and who is "taking". To be versatile, you'd both give it and take it. But what you're giving and taking is not "defined". You can be an oral top (you give your cock and someone else takes it into his mouth) but an anal bottom (you take a cock in your ass...).

I generally think of it more as a "state of mind". You could also think of it in terms of servicing vs. being serviced. In truth, it is just a label, and no one label fully defines who someone is.
