I have a couple suspensories -- they used to be sold in drug stores for extra support for surgery and injuries -- and I love them. One of mine has leg straps like the picture you posted, and the other is just the pouch.

Suspensory is most commonly used after hernia surgery for support. Had an extensive hernia repair a year ago. Dr had be get a suspensory to have on hand when I got out of surgery to go home. Yes, let me know. Never tried it. I like to play like this before a mirror because you can see the pleasure of the guy.
