Now that I have your attention I want to recommend a film that has all of this -- Shortbus, directed by John Cameron Mitchell. This is a regular theatrical release film, not a porn film, though it is unrated. It is the story of a group of people, both straight and gay, whose lives intersect at a Brooklyn sex club called Shortbus. (The reference is to the short school buses used to transport "special needs" students to school.) The plot and character development are excellent and despite the sex, one reviewer remarked on the "sweet midwestern sensibility" of the film. It is very sympathetic to the characters and their tribulations. For our purposes though, you should know that the film contains scenes of auto-fellatio, masturbation, two and three way gay sex, enthusiastic and very acrobatic straight sex, straight s&m, lesbian sex, orgies straight and gay and more. Underwear is usually not prominent (it's usually off), but there are scenes of the male characters in their underwear and a rather tame pool scene of two guys in speedos trying to seduce each other. The most notable underwear scene is one of a straight guy talking to his wife while seated at table with his erect cock and balls hanging out of his boxers.
Despite the above description, sex takes up maybe 10-15% of the film. It's a serious dramatic effort. Mitchell's aim is to show that Hollywood's treatment of sex is shallow, dirty-minded, and hypocritical and that real sex can be incorporated into a dramatic film (this one is actually fairly funny), to advance the plot and not be purely exploitive like a porn film.
I saw it at a local multiplex. If it comes to a theater near you, go see it! Unfortunately I suspect it will never be released in some parts of the USA.
