I never thought to try it until you asked! I suspect that there is always the risk of damage, whether pumping your penis or your balls -- too much suction, or too long, and I would think there could be damage. I tried it, with light suction, and it felt a little uncomfortable.

I tried again, this time without the rubber ring (it was a challenge to squeeze my balls through the hole in the centre of the rubber ring). It felt better without the rubber ring. My ball sac definitely gets stretched, moreso to fill the entire diameter of the tube than to stretching in length.
